In addition to submitting their contact information, registrants will also need to provide a recent photograph of the applicant as well as information about their need for a scooter. Monthly winners will be able to choose from one of three scooters, including the Golden Technologies Buzzaround Lite, the Pride Go-Go LX 3-Wheel, and the Drive Medical Spitfire Scout 4.
